I would ground the shield in one place to REAL ground, and have a "floating 485 ground" wire inside the cable.

It is, unfortunately a fact that many appnotes/datasheets rather than showing the safe and correct way show the cheap way just to sell chips "see how cheap our solution is" (RC reset as case in point).
