PS. the XA reset - except the short time, the output high voltage of supervisor is below the XA /RSThigh input threshold (and there is no reason to reset at 4V if XA operates down to 2.7V but that's minor) - I don't see the 3rd problem (low impedance and decoupled VCC/GND paths? - but that's no puzzle).
