Do you have the answer?
My answer is that you need bypass caps. on the MAX821 and XA chips. You need to remove the GND from the SRT pin on the MAX821 chip (1 msec is too short to wait for reset on power up) and you need a 100 kohm resistor to GND on the RESET line.
